Waiver

I hereby agree to participate in the elite basketball skills clinic hosted by Daniel James Basketball LTD. I acknowledge that there are inherent risks associated with participating in basketball activities, including but not limited to physical injury.

I understand that Daniel James Basketball LTD and its staff are not liable for any injuries or damages that may occur during the clinic. I release Daniel James Basketball LTD and its staff from any liability for any injuries, damages, or losses that may occur as a result of my participation in the clinic.

I also understand that I am responsible for my own medical and insurance coverage in case of injury. By signing this waiver, I certify that I am physically fit to participate in the clinic and assume all risks associated with participation.

I hereby authorize the use of photography and videography capture during my participation in the Daniel James Skills clinics, camps and special events in which I grant permission to use all materials for promotional purposes, including but not limited to website usage, advertising/marketing materials, and all social media platforms.

I acknowledge that my likeness may be included in these materials, and I consent to their use in all things related to Daniel James Basketball LTD.

I understand that by attending the elite basketball skills clinic, I am committing to upholding the values of integrity, respect, and inclusivity. I agree to treat the space and all participants with the utmost respect, refraining from any form of discrimination based on race, sex, gender, or any other characteristic.

I understand that this clinic is intended to be an inclusive and safe space where everyone, regardless of background, should feel welcome and comfortable. I will strive to be a team player, work collaboratively with others, and support my fellow participants in their growth and development.

I acknowledge that any behavior that goes against these values, including but not limited to harassment, bullying, or discrimination, will not be tolerated and may result in removal from the clinic.

By signing below, I affirm my commitment to attending the elite basketball skills clinic with integrity, respect, and a positive attitude towards all participants.
